Your Clinical History is a very important part of your outpatient surgery and our first step toward ensuring it is both a comfortable and stress free experience. Please complete your Clinical History either online or by the 5 page paper form provided by your surgeon. If you choose to complete the form online, you may be called regarding any instructions or concerns we may have. If you complete the paper form, please return it to our center as soon as possible by mail or fax. We will contact you by phone after we have reviewed the form. We will explain your surgical process and address any questions or concerns during the call. You may be interviewed by a nurse-representative of the Anesthesiology Department at our Center, who will ask additional questions about your medical history or physical condition.

You may call the center between the hours of 9:00 am and 4:00 pm, up to three weeks prior to the surgery. A nurse may also complete the Clinical History form with you over the phone when you call the center.

This Clinical History will be used by our Center to make the best possible decisions concerning your healthcare. Any laboratory tests ordered by your surgeon or anesthesiologist will be explained during your preoperative phone call.
